1. M.D (Ayurveda) Kayachikitsa awarded by a recognized University.

2. “A” Class Registration in the Travancore Cochin Medical Council.

Candidates should apply in the new application form, which is specially designed for processing through Optical Mark Reader (OMR) also.

         The cost of application form is Rs.10 (Ten).  Cost of application for SC/ST candidates is R.5 (Five) only.  Application forms can be obtained from all Post Offices in the State and from the Offices of Keala Public Service Commission.  The application form should be filled up in accordance with the instructions in the brochure which is supplied along with the application form.  Candidates should fill up both sides of the application form.   No documents should be enclosed with the application.   Copies of documents to prove Educational Qualification, Community, Age etc. should be submitted as and when called for by the Commission.  Applications which are not in the aforesaid form and those which are not filled up in accordance with the instructions in the brochure will be summarily rejected.

Application Form: The application form is a single sheet of paper specially designed for processing  through  Optical Mark Reader(OMR) also.  Candidates have to fill up both sides of the application.  Each application form contains  a unique eight digit barcode on side A.  This Bar Code number is unique for each application.  The candidate has to quote this number for further enquiries.

        The application forms will be available in the Post Offices inside the State and Offices of the Kerala Public Service Commission.

       The cost of the application form is Rs.10.  The cost of the application form for SC/ST candidates is Rs.5.  An information brochure will be supplied with the application form.  Candidates should read the instructions carefully before filling up the forms.  Candidates should not enclose any documents along with  the application.

The applicant shall send  his application in the cover supplied with the application    form.  Applications should be submitted in the form devised and printed by the Kerala Public Service Commission.  Applications otherwise submitted will be summarily rejected.

1. The name of the post, the Catergory Number and the Department of the respective post should be clearly noted at the top of the envelope.
